You know the move. Two fingers on your jawline, pulling back the extra skin a millimeter or two.
Suddenly, the lower face smooths out. The jawline looks sharp again. And the thought creates itself: “If I could just tuck this one spot, I’d be fine.”
In our Nashville office, this is the most common starting point for a consultation. Patients come in asking specifically for neck lift surgery, convinced that the neck is the only problem. It makes sense why. A neck lift sounds manageable. It sounds like a quick edit. A facelift, on the other hand, sounds like a major production.
But here is the reality of facial anatomy: the face and neck are a package deal. They age together, they slide down together, and usually, treating one without the other creates a mismatch.
If you’ve been researching neck lift vs. lower facelift and trying to figure out which camp you fall into, let’s look at the actual mechanics of how this works.
Think of a neck lift as a specialist. It has a very strict jurisdiction of the area between your chin and your collarbone.
This surgical procedure is excellent for specific, isolated issues. It handles the vertical neck bands (those cords that pop out when you speak), or clears up the stubborn pocket of excess fat that creates a double chin, regardless of your weight. It’s also the answer for turkey neck—that central area of loose neck skin that has lost its snap.
However, a neck lift focuses only on the neck area. It stops abruptly at the border of the jaw.
We typically recommend this cosmetic surgery for younger patients, usually people in their late 30s or early 40s. These patients often have great skin quality and zero jowling, but just need a little contouring to sharpen the angle. For them, simply removing excess skin or tightening the muscle in the neck area works perfectly.
But if you have jowls, that little bit of softness breaking up your jaw line, a neck lift alone will not help. In fact, if you tighten the neck while leaving sagging skin on the face, you can actually make the aging in the face look more obvious by contrast.
The lower facelift procedure is the architect.
Despite the name, a lower facelift isn’t about looking "pulled." It is about structural restoration. Where the traditional facelift tackles the entire face, the lower facelift addresses only the lower third of the face, smoothing out deep creases (marionette lines) and lifting the cheek tissues that have drifted downward.
Most patients who want a truly defined jawline are usually looking at a lower facelift. It picks up the drooping skin around the mouth and jaw and restores it to where it used to be before the aging process brought everything down.
Dr. O’Connor often compares it to making a bed. If the top sheet (your face) has slid down, tucking in the bottom sheet (your neck) really tight won’t smooth out the rumpled sheets on top. You have to smooth both layers to get that seamless, peaceful result.
This is why many patients who walk in wanting a neck lift actually end up happier with a combined approach.

While you might see ads for a mini facelift under local anesthesia, we find that comprehensive, lasting work requires the control and safety of being fully asleep. Once you are under, your board-certified plastic surgeon makes small incisions around the treatment area (depending on what procedure you opt for, neck lift surgery, lower facelift, or both cosmetic procedures) to skillfully remove loose skin and tighten up the rest just enough to leave you looking younger with less visible signs of aging.
As for visible scarring, it’s a major worry for everyone. But in skilled hands, facelift incisions are artfully camouflaged. They trace the hairline and the natural curves of the ear, becoming nearly invisible once healed. For the neck lift, a tiny incision is hidden under the chin to access the muscles and remove excess skin.
Recovery is often the biggest mental hurdle, but physically, it is certainly manageable. It's important to keep realistic expectations, though, as the recovery period does demand rest, proper aftercare, and staying hydrated.
It is crucial that you quit smoking well before surgery—nicotine restricts blood flow, which is vital for safe healing.
There are several factors that go into a great result from a neck lift, lower facelift, or a combo of both surgical procedures: your bone structure, your medical history, and your overall health. But the biggest variable is your surgeon.
